Our providers agree to provide timely return of all clinical data; consultations, discharge plans or compensation and pension (C&P) exams to Humana Veterans for any veteran who has received authorized services from the provider.
 
Providers also agree to submit consultation reports, discharge plans (to include any relevant pathology reports) or C&P exams to Humana Veterans within 10 days of the date of the veterans completed appointment.  Consultation reports, discharge plans or C&P exams should be faxed to Humana Veterans at 1-866-836-4991.  Our fax number is also listed on your authorization form.
 
Diagnostic data (radiology reports) must be signed electronically and returned to Humana Veterans within 48 hours.  Diagnostic radiology providers are encouraged to send a CD of images to the Radiology Department of the referring Veterans Affairs Medical Center (VAMC), (as listed on your authorization form).
 
Abnormal test results are requested immediately or within 24 hours.
